Job summary

EasyHealth is growing its Nurse Practitioner network in Portland, OR, offering in-home preventive care with flexible scheduling and autonomous practice. You’ll visit members in their homes, perform comprehensive health assessments, and connect them with ongoing care.

This role emphasizes meaningful patient interactions outside traditional clinics. Ideal candidates hold NP licensure in Oregon and/or Washington, are board-certified, and have experience in Family or Primary care settings.

Qualifications

• Active Oregon and/or Washington NP license required.
• Board-certified Nurse Practitioner.
• Experience in Family Medicine, Primary Care, Internal Medicine, Urgent Care, or geriatrics preferred.
• Ability to conduct comprehensive in-home health assessments and identify care gaps.
• Willingness to travel locally for scheduled in-home visits.
• Strong communication and clinical decision-making skills.

Looking for a way to practice that gives you more autonomy, meaningful patient interaction, and flexibility outside the traditional clinic setting? EasyHealth is growing our Nurse Practitioner network in Portland, Oregon in surrounding areas. We're looking for experienced, compassionate NPs who want to make healthcare more accessible; one home at a time.

What You'll Do

You’ll meet members where they are, literally. During scheduled in-home visits, you’ll conduct comprehensive preventive health assessments that help identify care needs, close important gaps, and connect members to appropriate ongoing care. This is not a traditional primary care practice or packed clinic schedule. You’ll have the opportunity to focus on the member in front of you and deliver a thoughtful, high-quality assessment.

We're Looking for NPs Who

• Hold an active Oregon and/or Washington Nurse Practitioner license
• Are nationally board certified
• Have Family Medicine, Primary Care, Internal Medicine, Urgent Care, Geriatrics, or similar clinical experience preferred
• Are comfortable conducting comprehensive health assessments, identifying care gaps, and working with adult and older-adult populations
• Have strong assessment and clinical decision-making skills
• Are comfortable traveling locally for scheduled in-home visits
• Enjoy working independently while being part of a supportive clinical organization
• Bring warmth, professionalism, and excellent communication skills into every member’s home
