Ultra Health Providers (UHP) is seeking a Nurse Practitioner to deliver comprehensive medical care to residents in skilled nursing and senior living settings. This role offers clinical autonomy, collaborative support from an interdisciplinary team, and a focus on patient-centered outcomes.
You will perform preventive, urgent, and routine care, order tests, prescribe therapies within scope, and document in the EMR. Requires active NP license and national certification; DEA registration is a plus.

As a Nurse Practitioner with UHP, you will play a vital role in delivering comprehensive medical care to residents in skilled nursing and senior living settings. This role offers the opportunity to work independently, manage complex medical conditions, and build strong, trusted clinical relationships — all while being supported by a collaborative interdisciplinary team.
You will serve as a consistent clinical presence within your assigned facilities, providing proactive, patient-centered care and responding to urgent medical needs to ensure continuity, quality, and dignity in treatment.
