Optum Home & Community Care (Senior Community Care) is hiring a Per Diem Nurse Practitioner or Physician Assistant to provide high-quality, coordinated care to Medicare and Medicaid patients in nursing homes, assisted living, senior housing, and occasional private residences.
In this role, you’ll deliver primary care services, manage chronic and acute conditions, support care transitions, and partner closely with facility staff, families, and physicians to improve outcomes and reduce unnecessary hospitalizations.
