Job summary

Ultra Health Providers is seeking a part-time Nurse Practitioner to deliver primary, urgent, and preventive care to residents in skilled nursing and assisted living communities. You will work a Monday–Friday schedule on-site, collaborating closely with nursing staff and physicians to optimize patient outcomes.

The ideal candidate holds an active TN NP license, current national certification (AANP or ANCC), and 1–2 years of primary care or geriatric experience.

Qualifications

• Minimum 1–2 years of experience in primary care, geriatric, or long-term care settings preferred.
• Active and unrestricted Nurse Practitioner license in Tennessee.
• Current national board certification (AANP or ANCC).

Responsibilities

• Deliver primary, urgent, and preventive care to residents in skilled nursing and assisted living settings.
• Conduct admission assessments, physical exams, medication reviews, and depression screenings.
• Collaborate with attending physicians, nursing staff, and facility leaders to coordinate care.
• Counsel patients and family members on treatment options and changes in health status.
• Manage clinical documentation accurately and in compliance with facility and state standards.

Role Overview
