Job Summary

An Advanced Practice Provider is a licensed and board‑certified nurse practitioner or physician’s assistant specifically assigned to each Longevity Health Plan member for the purpose of executing the Institutional Special Needs Plan Model of Care. The provider will effectively medically manage a panel of enrolled nursing home members in partnership with the primary care team and deliver the Longevity Health Plan Model of Care. The APP conducts a comprehensive assessment, develops and delivers individualized plans of care, attends nursing facility care team meetings, communicates frequently with residents’ families, and ensures the care team understands the member’s goals of care and needs. The APP focuses on treating in place, uses clinical practice guidelines to guide treatment decisions, and effectively documents health status, functional status, and diagnoses. The APP is supported by the APP Clinical Manager, Market Director of Clinical Services, and the local medical director who provide education and guidance on best practices to care for members.

Key Tasks and Responsibilities

Provides collaborative primary care and services to a panel of Longevity Health Plan members, treating in the most appropriate setting based on their goals of care. Executes and delivers Longevity Health Plan Model of Care activities, develops and implements advanced care planning, embraces and executes a treating‑in‑place model, facilitates health plan clinical operations—prior authorization, Part A and B services, medication reviews, HEDIS and STARs quality gap closure, and promotes membership growth. Serves as a trusted advisor to members, loved ones, nursing home staff, and member advocates; educates care teams on member needs and early identification of changes; notifies APP and telehealth partners of client changes. Delivers effective and frequent communication and coordination with PCPs, loved ones, and care team members. Supports overall market goals and balances member caseload demands with team support needs. Promotes Longevity Health Plan with members, loved ones, and nursing facilities and other stakeholders. Is able to function autonomously, accountable for practice, and establishes meaningful relationships in the nursing home environment. Performs other related duties as assigned.

There are no supervisory responsibilities for this position.

Credentials & Coverage

State‑unrestricted licensed Advanced Practice Nurse or Physician’s Assistant with national board certification (AANP, ANCC or AACN). DEA license required. Must have an active NPI and active Medicare provider number and be eligible to participate in Medicare as a provider.

Insurance and Driving

Valid state driver’s license with a good driving record and proof of automobile insurance required. Auto liability insurance declaration page with minimum limits of $100,000/$300,000 for bodily injury and $100,000 for property damage with a combined single limit of $300,000. Proof of coverage must be provided annually.

Education and Training

Master of Science in Nursing as a Nurse Practitioner from a nationally accredited program, or a Master’s Degree from a nationally accredited program.

Experience and Qualifications
  • Minimum six months experience as a Nurse Practitioner or Physician Assistant.
  • Experience working in the nursing home environment is strongly desired.
  • Computer/Technology proficiency required.
  • Ability to engage and communicate effectively with physicians, patients, and family members.
  • Excellent people skills.
  • Self‑directed but functions well in a team environment.
  • Excellent clinical skills—assessment, diagnosis, treatment, prescribing specifically for the chronically ill population.
  • Demonstrated critical thinking, organization, and prioritization skills.
