Job summary

Optum Home & Community Care’s HouseCalls program seeks clinicians to provide preventive care directly in patients’ homes. This non-prescribing, field-based role emphasizes education, assessments, and care coordination to improve outcomes.

You’ll conduct in-home health assessments, perform screenings and document diagnoses, and share findings with primary care providers. The schedule runs 8am–8pm across a 7-day week with no on-call, weekends, or holidays.

Qualifications

• Active, unrestricted state license (or ability to obtain)
• NP (ANCC/AANP) or PA (NCCPA) certification
• Valid driver’s license and reliable transportation

About the Role

Required Qualifications

Preferred

• 1+ year of clinical experience (geriatrics, family, or home health)
• Comfort with EMR systems/technology
• Experience working with Medicare or senior populations
