Job summary

Rainbow Community Care in Kentucky seeks a Nurse Practitioner to provide Hospice and Palliative care in inpatient and field settings. The NP collaborates with the Medical Director and interdisciplinary teams, performs patient assessments, orders tests, and participates in home visits and inpatient evaluations.

You will educate patients and families, document care, and contribute to care planning and patient comfort.

Qualifications

  • Minimum one year of nurse practitioner experience preferred.
  • Hospice and palliative care experience preferred.
  • Valid driver's license, reliable auto, and current auto insurance required.
  • Ability to relate to people of diverse cultures and backgrounds.
  • Strong teaching and leadership skills required.
  • Current CPR certification.

Responsibilities

  • Performs bedside evaluations, histories, physicals, orders, and discharge instructions.
  • Coordinates with social workers and case managers for hospice/palliative care.
  • Orders, performs, and interprets labs and radiology tests as appropriate.
  • Prescribes medications, orders, and durable medical equipment as indicated.
  • Educates staff, patients and families on disease management and confidentiality.
  • Maintains accurate patient records and supports clinical documentation protocols.
  • Demonstrates empathy and sensitivity to patients and families.
  • Prepares concise clinical notes and uses laptop charting.

Purpose of the Position

The Nurse Practitioner works in collaboration with the Medical Director, Hospice Physicians, Hospice and Palliative Care Teams to provide Hospice and Palliative care services to patients and families in a variety of settings. The Nurse Practitioner will perform procedures for which he or she is privileged to do and will document assessments, evaluations, consultation reports and progress notes. Home visits/evaluations, and inpatient evaluations will be part of the Nurse Practitioners role.

Key Responsibilities

  • Performs clinical care which includes bedside evaluation of patients, patients’ histories; physical examinations; radiological and laboratory interpretations; follow-up inpatient care; discharge orders, patient instructions and education; and participation in case management.
  • Provides services related to hospice and palliative care, assesses patient needs and anticipates service requirements prior to discharge to home. Coordinates with social workers, case managers and applicable ancillary staff regarding hospice and palliative care services.
  • Orders, performs, and interprets laboratory and radiology tests as appropriate.
  • Prescribes medications including controlled substances to the extent delegated and licensed. Orders, treatments and durable medical equipment as indicated. Performs other therapeutic measures as indicated.
  • Educates clinical staff, patients and families on applicable disease management and prevention strategies. Adheres to the practice of confidentiality regarding patients, families, staff and the organization.
  • Prepares and maintains accurate patient records, charts and documents to support sound hospice and palliative practices and reimbursement for services provided. Assists in the development of clinical documentation protocols.
  • Must demonstrate empathy and sensitivity to the needs of the patient and family.
  • Prepares concise, legible timely clinical notes, utilizing agency guidelines. Becomes trained on laptop charting.

Education/Experience

  • A minimum of one year’s nurse practitioner experience is preferred.
  • Previous Hospice and Palliative Care experience is preferred.

  • Ability to relate to people of diverse cultures and ethnic backgrounds.
  • Must demonstrate maturity, initiative, tact and good judgment in dealing with staff, patients and families.
  • Strong teaching and leadership skills required.
  • Clinical documentation experience preferred.
  • Current Cardiopulmonary Resuscitation certification.
  • Ability to be self-directed and handle stressful and emergency situations.
  • Leadership qualities and the ability to maintain positive interpersonal relationships
  • Ability to mentor students and provide input for evaluations.
