The Nurse Practitioner (NP) or Physician Assistant (PA) will perform weekend rounds and deliver preventative care in skilled nursing facilities. The role is hybrid (on-site visits plus remote work) with support from a dedicated scribe team for documentation.

Carease Health is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to do Weekend rounds and deliver high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

Success in this role depends on building strong relationships with the facility, maintaining a patient-centered approach, and being a collaborative team player.
