Qualifications

• DEA license required.
• Experience in home health, hospice, or facility-based care.
• Strong communication and clinical skills.
• Ability to work independently across diverse care environments.

Responsibilities

• Provide primary medical care in home, facility, and telehealth settings.
• Perform exams, wellness visits, and manage chronic/acute conditions.
• Order and interpret diagnostic tests and labs.
• Prescribe and manage medications and individualized care plans.
• Coordinate care with interdisciplinary teams.
• Promote preventive care and patient education.
• Participate in after-hours or on-call rotation.

Responsibilities:

• Provide primary medical care services in home, facility, and telehealth settings
• Conduct physical exams, wellness visits, and manage chronic and acute conditions
• Order and interpret diagnostic tests and labs
• Prescribe and manage medications and individualized care plans
• Coordinate care with interdisciplinary teams and community partners
• Promote preventive care, patient education, and continuity of care
• Participate in after-hours or on-call rotation as needed

Qualifications:

• DEA license required
• Experience in home health, hospice, or facility-based care required
• Strong communication, decision-making, and clinical skills
• Comfortable working independently across diverse care environments

Preferred:

• Facility-based or post-acute care experience
• Familiarity with telehealth technology and EMRs
