Job summary

Colorado Kidney Care, the largest private nephrology practice in the Denver area, seeks a full-time Advanced Practitioner (NP) to join the Lakewood area team.

The role involves caring for CKD and ESRD patients, rounding in dialysis units, performing chair-side visits, charting, and coordinating with physicians. Must travel to various work locations with reliable transportation. Competitive pay and comprehensive benefits offered.

Qualifications

  • Must have completed an NP degree from an accredited program.
  • Must be board-certified.
  • Current BLS certification.
  • Colorado prescriptive authority or active DEA number.
  • New grads considered if RN with solid clinical work history in related areas.

Responsibilities

  • Round, assess and evaluate ESRD patients in dialysis units; chair-side visits and charting.
  • Fill prescriptions chair-side and by phone; coordinate with referring physicians.
  • Assist with tests when dialysis staff requests help; complete annual H&Ps on ESRD patients.
  • Evaluate plan of care and adjust as indicated; perform in-person and televisits.
  • Educate patients on medications, labs, diet and follow-up plans.
  • Coordinate referrals for dialysis, transplant, access and dietary services.

We are Colorado Kidney Care, the largest private nephrology practice,with offices across the Denver metro area, and we are looking for onefull time exemplary Advanced Practitioner to join our team!

We are seeking a qualified NP candidate to join our team who willwork in the Lakewood area. This position will care for ColoradoKidney care office patients as well outpatient dialysis patients. Allcandidates must be able and willing to travel to different worklocations regularly; reliable transportation is required.

We offer competitive pay range $113,000-$115,000 annual salarycommensurate with experience and excellent benefits including:

We employ a collaborative model of care, with responsibilities including:

Responsibilities and Duties:

  • To round, assess and evaluate as directed on each ESRD patient inthe dialysis units to which you have been assigned. ESRD patientduties include: chair-side visits and charting of each visit,filling of prescriptions both chair-side and by phone, contactingreferring physicians regarding clinical issues, assisting witharrangement of tests when assistance is needed from the dialysisstaff to clarify medical issues, and completing annual History andPhysicals on ESRD patients as needed.
  • Evaluate plan of care, adjust as indicated.
  • Perform in-person office visits and televisits on follow-uppatients with CKD.
  • Perform office consults on specified hospital follow-ups who needprompt office follow-up.
  • Patient education both in office and via phone calls tocommunicate physician instructions regarding; medication changes,requested diagnostic follow-up/labs, diet changes, next visit, etc.
  • Diabetes education including: diet, exercise, monitoring,medication management, problem solving, coping strategies, andco-morbid risk reduction strategies.
  • Assist with patient referrals and coordination of care regardingdialysis, transplant, access, and dietary services.
  • Maintain contact with patients to ensure anticipated therapeuticobjectives are being met (i.e. weight, BP, diet, labwork) andcommunicate with treating nephrologist regarding objectives.
  • Assist physicians with EHR documentation by annotating physicianscompletely on all interactions and patient care issues regardingtheir patients.
  • Entry, Review, Refill and Maintenance of medications in EHR
  • EHR software patient care data entry must be accurate and timely;you are expected to document all patient care interactions andpertinent medical record information in the EHR charts. Thisincludes both in person office visits as well as phone calls withthe patient.
  • Answering and triage of phone calls from patients seekingadvice/clarification that the office staff cannot answer.
  • Maintenance and upkeep of incoming messages via EHR and email;this includes timely responding and follow up
  • Cross coverage of other duties as needed, and other duties as assigned
Experience and Skills:

Experience and Requirements:

Must have completed an NP degree from an accredited program

Must be board-certified

Current BLS certification

New grads considered with solid clinical work history as RN inChronic Disease/ICU/med-surg/cardiology/dialysis or other related,intense specialty

Must have prescriptive authority in the state of Coloradoor have an active and valid DEA number.

Excellent interpersonal, organizational and communication skills required

Strong computer skills and ability to learn new software quickly andeasily. Knowledge and experience with EHR systems is preferred.

Communicate verbally and in writing in a precise, clear, andprofessional manner

Maintain a high level of professionalism in appearance, behavior, and communication

Compassion for patients and the desire to help them

Possess and display a high level of patience in all situations

Effectively and professionally prioritize and multitask in a fastpaced, sometimes stressful, environment

Possess an intermediate skill level with Microsoft Word, Excel, and Outlook

Maintain a high level of organization and attention to detail

Valid Driver's License and good MVR
