Job summary

Minnesota Hospice is seeking a Nurse Practitioner to provide advanced clinical care, rely on strong communication, and support patients and families through the hospice journey. You will collaborate with the interdisciplinary team to ensure timely assessment, coordination, and delivery of comfort-focused care aligned with goals of care.

The role requires licensure as an RN and NP in Minnesota, board certification, and at least three years of clinical experience in hospice or palliative care.

Qualifications

  • Masters or Doctoral degree in Nursing from accredited nursing program.
  • Current, active licensure as a Registered Nurse (RN) and Nurse Practitioner (NP) in Minnesota.
  • Board certification as a Nurse Practitioner in an appropriate specialty area.
  • Minimum of three years clinical experience as a practicing NP, hospice or palliative care preferred.
  • Current DEA registration and ability to prescribe medications in accordance with state and federal regulations.
  • Current CPR certification required.

Responsibilities

  • Assess the physical, functional, and psychosocial status of hospice patients through comprehensive evaluations.
  • Make clinical recommendations to the Medical Director regarding patient eligibility for hospice certification and recertification.
  • Conduct required face-to-face visits to ensure continued medical eligibility in accordance with regulatory guidelines.
  • Provide direct medical care to patients within the scope of NP practice, focusing on symptom management, comfort, and quality of life.
  • Document all patient encounters, assessments, and clinical decisions accurately in the EHR.
  • Communicate changes in patient condition, prognosis, and care needs with the interdisciplinary team promptly.
  • Collaborate with nursing, social work, chaplaincy, and other team members to develop individualized plans of care.
  • Participate in on-call rotation to ensure timely clinical support.
  • Review and interpret clinical data to guide care planning.
  • Support advance care planning discussions and ensure care aligns with patient and family goals.
  • Provide education to patients, families, and caregivers regarding disease progression and end-of-life expectations.
  • Ensure compliance with hospice regulations and policies.
  • Assist with care transitions including admissions, recertifications, and discharges.
  • Maintain professional relationships to promote continuity of care.

Job Summary

The Nurse Practitioner utilizes advanced clinical expertise, strong communication skills, and a high level of compassion to support patients and their families throughout the hospice journey. This role serves as a key liaison for continuously evolving medical information, ensuring timely assessment, coordination, and delivery of appropriate care. The Nurse Practitioner collaborates closely with the interdisciplinary team to promote quality outcomes, maintain patient comfort, and uphold the goals of care in alignment with hospice philosophy.

Duties/Responsibilities
  • Assess the physical, functional, and psychosocial status of hospice patients through comprehensive evaluations.
  • Make clinical recommendations to the Medical Director regarding patient eligibility for hospice certification and recertification.
  • Conduct required face-to-face visits to ensure continued medical eligibility in accordance with regulatory guidelines.
  • Provide direct medical care to patients within the scope of Nurse Practitioner practice, focusing on symptom management, comfort, and quality of life.
  • Document all patient encounters, assessments, and clinical decisions accurately in the electronic health record (EHR).
  • Communicate changes in patient condition, prognosis, and care needs promptly and effectively with members of the interdisciplinary team.
  • Collaborate with nursing, social work, chaplaincy, and other team members to develop and adjust individualized plans of care.
  • Participate in as-needed patient visits to support ongoing clinical oversight and patient needs.
  • Participate in a provider on-call rotation to ensure timely clinical support and responsiveness to patient needs.
  • Review and interpret clinical data, including medications, lab results, and disease progression, to guide care planning.
  • Support advance care planning discussions and ensure care aligns with patient and family goals.
  • Provide education to patients, families, and caregivers regarding disease progression, symptom management, and end-of-life expectations.
  • Ensure compliance with hospice regulations, organizational policies, and best practice standards.
  • Assist with coordination of care transitions, including admissions, recertifications, and discharges as appropriate.
  • Maintain strong, professional relationships with patients, families, referral sources, and interdisciplinary team members to promote continuity of care and a high level of trust and collaboration.
Required Skills/Abilities
  • Knowledge of appropriate HIPAA and confidentiality
  • Knowledge of the Medicare CoPs
  • Maintain accurate documentation
  • Able to work as both a part of a team and independently
  • Possesses great organizational skills
  • Excellent time management skills.
  • Valid driver’s license and auto insurance
  • Knowledge of Microsoft products i.e. Excel, Word, etc.
Education and Experience
  • Masters or Doctoral degree in Nursing from accredited nursing program
  • Current, active licensure as a Registered Nurse (RN) and Nurse Practitioner (NP) in Minnesota
  • Board certification as a Nurse Practitioner in an appropriate specialty area
  • Minimum of three (3) years clinical experience as a practicing Nurse Practitioner, hospice or palliative care experience preferred
  • Current DEA registration and ability to prescribe medications in accordance with state and federal regulations
  • Current CPR certification requried
Physical Requirements
  • Able to regularly lift and move up to 50lbs
  • Regularly operate and assist with physical and mechanical transfers
  • May have prolonged periods of sitting at a desk and working on a computer
  • Personal protective equipment provided, including masks
