Community Health Center in Fort Dodge, IA is seeking a full‑time Family Practice Nurse Practitioner to provide comprehensive primary care to patients of all ages in a community‑based, patient‑centered setting. Under physician supervision, the FNP will diagnose and treat acute and chronic illnesses, manage care plans, prescribe medications, and coordinate with a multidisciplinary team.

Salary Range: $110,000.00 To $170,000.00 Annually
The Family Practice Nurse Practitioner (FNP) provides comprehensive primary care services to patients of all ages in a community-based, patient-centered environment. Working within the scope of practice and under the collaborative supervision of a physician, the FNP promotes health, prevents illness, manages chronic conditions, and delivers high-quality, culturally competent care. This position is key in fulfilling the FQHC’s mission to improve access to quality health care for underserved populations.
Education and Experience:
Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP).
Minimum of 1-2 years’ experience in primary care or family practice preferred; new graduates with strong clinical rotations may be considered.
Licensure and Certification:
Current, unrestricted license to practice as an Advanced Registered Nurse Practitioner (ARNP) in the state of Iowa.
Certification as a Family Nurse Practitioner by AANP or ANCC.
DEA registration and state prescriptive authority required.
BLS certification.
Knowledge of primary care principles, chronic disease management, and community health.
Strong communication skills with the ability to build rapport with diverse patient populations.
Commitment to patient-centered, trauma-informed, and culturally competent care.
Proficiency with EHR systems; experience with Epic a plus.
Ability to work independently and collaboratively within a multidisciplinary team.
Bilingual skills preferred but not required.
Clinical environment in an outpatient community health center.
May require occasional evening hours to meet patient needs.
Ability to sit, stand, walk, and perform repetitive motions for extended periods.
Occasional lifting of up to 25 pounds may be required.
