Job summary

Allina Health Faribault Clinic in Minnesota seeks a Nurse Practitioner to join its outpatient Primary Care team. You will provide care to patients of all ages in a collaborative physician-led environment, with 36 patient contact hours per week and limited call; EMR is Excellian, and clinic hours are Mon-Fri 7am-5pm.

Qualifications

• Master's degree completion of accredited Nurse Practitioner program.
• Minimum 2 years experience post Nurse Practitioner training strongly preferred.
• Licensed Registered Nurse - MN Board of Nursing required upon hire.
• Certified Nurse Practitioner - ANCC required certification within specialty upon hire.
• ACLS/BLS Tier 2 required within 180 days of hire.
• DEA license required upon hire.
• Valid Driver’s License required upon hire; MN/WI residents must obtain license in state of residence within 60 days if applicable.

Responsibilities

• Provides clinical care for patients; completes comprehensive patient assessments.
• Determines diagnosis and treatment of illness in collaboration with attending physician.
• Orders and interprets diagnostics in tandem with the physician.
• Prescribes medications and therapies; manages chronic illness and wellness care.
• Educates patients and refers to specialty care when needed.
• Collaborates with the primary care team and other services; acts as liaison with outside agencies.
• Performs other duties as assigned.
