Job summary

Circadia Health is seeking an on-site Nurse Practitioner in Detroit, MI to evaluate residents, act on predictive health signals, and mentor facility teams in using Circadia to intervene earlier. You will blend direct patient care with technology-driven bedside integration through hands-on training and live demonstrations.

Applicants should be licensed as NPs in Michigan with at least two years of experience serving complex adult populations, and be comfortable traveling locally.

Qualifications

• Licensed Nurse Practitioner in Michigan (ANCC-certified or equivalent).
• Eligible for Medicare participation; company bills on your behalf.
• Minimum 2 years NP experience with complex adult populations.
• Valid MI driver’s license; ability to travel locally.
• English required; startup environment experience preferred.

Responsibilities

• Direct patient assessments and adjust care plans.
• Train staff to integrate Circadia into clinical workflows.
• Interpret AI signals to prioritize high-risk residents and initiate interventions.
• Complete documentation on the same day.
• Perform high-volume rounds (30+ visits/day) across SNFs where applicable.
• Provide feedback to product/ops to improve workflows.

About the Role

Transform bedside care with AI. As Circadia’s on-site NP, you’ll see patients first, act on predictive health signals in real time, and coach facility teams to turn data into earlier interventions—reducing preventable readmissions. You’ll blend direct patient care with tech-to-bedside integration through hands-on training, live demos, and shadowed assessments.

What You’ll Do

• Direct patient care: Conduct initial and follow-up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers.
• Tech → bedside integration: Train and coach staff at the point of care, standardize use of Circadia into clinical workflows.
• Data into action: Interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent decline.
• Close the loop: Complete documentation/charts within the same day
• High-volume rounding: Efficiently complete visits across designated SNFs (typ. 30+/day where appropriate), focusing on new admits, facility-flagged patients, and Circadia alerts.
• Quality & product feedback: Surface field insights to improve workflows and tech integration in a clinical setting
• Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams.
• Regional flexibility: Cover sites within a 30–40-mile radius; for sites outside of that radius, mileage and lodging covered per policy.

How Success is Measured (KPIs)

• Visits: Target 30 patient visits/day
• Engagement: ≥80% Time to Acknowledgement Rate: <24h
• Documentation: Complete all notes using AI-powered tools within 24 hours of each encounter.
• Adoption & impact: Demonstrated workflow adherence at facilities; trend toward fewer falls/readmissions over time.

Must-Have Qualifications

• Certified and licensed Nurse Practitioner in MI; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid MI driver’s license; comfortable traveling locally.
• Thrives in a dynamic, evolving startup environment.
• English required

• Tech-savvy: Google Suite, multiple EMRs, Slack, AI transcription/tools.
• Spanish is preferred.

$140,000 - $180,000 a year

As a full-time Nurse Practitioner, you will be employed by Circadia Health, Inc. The anticipated annual base salary range for this full-time position is $140,000 - $180,000. The base range is determined by role and level, and placement within the range will depend on a number of job-related factors, including but not limited to your skills, qualifications, experience, and location.
