Job summary

Circadia Health is seeking an on-site Nurse Practitioner to see patients first, respond to real-time health signals, and coach facility teams to turn data into timely care interventions. The role covers Sun City and Phoenix areas within a 30–40 mile radius, with mileage and lodging covered per policy.

Qualifications

• Must be a licensed Nurse Practitioner in AZ with ANCC or equivalent.
• 2+ years NP experience with complex adult populations; able to practice independently.
• English required and clinical communication skills.

About the Role

Transform bedside care with AI. As Circadia's on-site NP, you'll see patients first, act on predictive health signals in real time, and coach facility teams to turn data into earlier interventions - reducing preventable readmissions. You'll blend direct patient care with tech-to-bedside integration through hands-on training, live demos, and shadowed assessments.

What You'll Do

• Direct patient care: Conduct initial and follow-up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers.
• Tech → bedside integration: Train and coach staff at the point of care, standardize use of Circadia into clinical workflows.
• Data into action: Interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent decline.
• Close the loop: Complete documentation/charts within the same day
• High-volume rounding: Efficiently complete visits across designated SNFs (typ. 30+/day where appropriate), focusing on new admits, facility-flagged patients, and Circadia alerts.
• Quality & product feedback: Surface field insights to improve workflows and tech integration in a clinical setting
• Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams.
• Regional flexibility: Cover sites within a 30-40-mile radius; for sites outside of that radius, mileage and lodging covered per policy.

How Success is Measured (KPIs)

• Visits: Target 30 patient visits/day
• Engagement: ≥80% Time to Acknowledgement Rate: <24h
• Documentation: Complete all notes using AI-powered tools within 24 hours of each encounter.
• Adoption & impact: Demonstrated workflow adherence at facilities; trend toward fewer falls/readmissions over time.

Must-Have Qualifications

• Certified and licensed Nurse Practitioner in AZ; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid AZ driver's license; comfortable traveling locally.
• Thrives in a dynamic, evolving startup environment.
• English required

• Tech-savvy: Google Suite, multiple EMRs, Slack, AI transcription/tools.
• Spanish is preferred.

As a full-time Nurse Practitioner, you will be employed by Circadia Health, Inc. The anticipated annual base salary range for this full-time position is $120,000 - $140,000. The base range is determined by role and level, and placement within the range will depend on a number of job-related factors, including but not limited to your skills, qualifications, experience, and location.
