Job summary

Optum Home & Community Care is expanding its nurse practitioner team to deliver in-home, no-nights, no-holidays care across assigned areas. The NP will join an interdisciplinary care team to assess, manage, and coordinate care for high-risk beneficiaries in home settings.

Responsibilities include comprehensive in-home assessments, acute and sub-acute management, and advanced care planning conversations, with patient and caregiver counseling. Travel to patients' homes is required.

Qualifications

  • Active ANCC or AANP certification for NP (Family, Adult, Geriatric, or ADAP Care)
  • Active, unrestricted license in the practicing state
  • Active and unrestricted driver’s license with reliable transportation
  • DEA licensure/prescriptive authority or ability to obtain post-hire

This position requires daily travel to patient homes within assigned area.

The Optum at Home program is a longitudinal, integrated care delivery program that coordinates the delivery and provision of clinical care of patients (beneficiaries) in their place of residence. The APC within OAH provides care to our highest-risk health plan beneficiaries and is part of an interdisciplinary team. Primary Responsibilities:
  • Conduct comprehensive in-home assessments, including:
    • Past medical history review, medication reconciliation, vital signs, and physical exam
    • Evidence-based screenings and point-of-care testing (as appropriate)
  • Manage members through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, interventions
  • Manage members during a short term sub-acute rehabilitation episode
  • Ensure accurate and complete documentation, including ICD 10 conditions
  • Communicate and collaborate with the interdisciplinary care team
  • Conduct advanced illness and advanced care planning conversations
  • Provide patients and caregivers with counseling and education

Required Qualifications:
  • For NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
  • Active, unrestricted licensure in the state you will be practicing in (or ability to obtain by start date) without current disciplinary actions or practice limitations
  • Active and unrestricted driver’s license
    • In addition to a driver’s license, access to reliable transportation
  • Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice)
  • Ability to gain a collaborative practice agreement, if applicable in your state
  • Ability to navigate varied environments; ability to sit/stand/kneel as needed to perform duties
Preferred Qualifications:
  • 1+ years of clinical experience as an APC (family, geriatric, or home health preferred)
  • Experience in geriatric medicine, long-term care, senior living or home care setting
  • Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
  • Understanding of Geriatrics and Chronic Illness
  • Understanding of Advanced Illness and end of life discussions
  • Proficiency with electronic medical records and technology

Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
