Job summary

Wellstar Health System is seeking a Nurse Navigator (NN), a licensed registered nurse with disease-specific knowledge. You will aid patients, families, and caregivers, using the nursing process to promote timely access to care and adherence to the care plan. The role involves in-person, telephonic, and electronic interactions, not bed-side care.

You will support navigation program goals, collaborate with leaders, and uphold clinical, regulatory, and safety standards across the cancer continuum.

Qualifications

• Associates, Diploma, or Bachelor's in Nursing; Bachelor's preferred
• All certifications required at hire (BLS, RN)
• Minimum 3 years nursing practice; 2 years disease-specific experience preferred
• Able to function in a fast-paced environment with strong time management and critical thinking

Responsibilities

• Act as subject matter expert in patient navigation and improve care access
• Coordinate care and documentation across multidisciplinary teams
• Provide patient-centered care with culturally sensitive approaches
• Participate in data collection and performance improvement initiatives
