Job summary

MEP Health in Phoenix is seeking a care coordinator to guide patients through complex care journeys in a fast-paced environment. You will coordinate appointments, referrals, and services across multidisciplinary teams, and ensure safe and timely transitions between care settings.

You will conduct clinical assessments, educate patients and families, address barriers to care to optimize outcomes, document care activities in EMRs, maintain safety standards, collaborate with EMS and hospital
