Job summary

McLaren Health is seeking an experienced RN Navigator to provide direct patient care and coordinate complex care for patients and families. The role eliminates barriers to timely care and supports flow through the health system, serving as the primary contact for navigated patients and collaborating with physicians, nurses, and ancillary staff.

The successful candidate will perform comprehensive nursing assessments, develop treatment plans, and deliver education to patients and families.

Qualifications

• Nursing degree from an accredited institution.
• State licensure as a registered nurse.
• Three years of experience directly related to the duties and responsibilities specified.
• For clinics with the Rural Health Designation, Basic Life Support (BLS) certification within 6 months of hire.

Responsibilities

• Provides direct patient nursing care for specified patients.
• Performs nursing assessments and triage of patient care needs for new and ongoing patients.
• Oversees the planning, scheduling, and implementation of day-to-day clinical activities and procedures for navigated patients.
• Central point of contact for all navigated patients; interaction with medical, nursing, and ancillary services.
• Provides education to patients, families, and significant others; acts as an information resource to students, health care professionals, patients, and the public.
