Collaborates with multidisciplinary teams to ensure seamless transitions, address barriers to care, and optimize health outcomes. Ensures effective communication among patients, providers, and community resources. Promotes continuous improvement in care delivery through patient feedback, program development, and ongoing professional education. ## Essential Functions• Provides individualized support, education, and information to patients and families throughout the care continuum, including disease process, treatment, and community resources.• Serves as the primary liaison and communication link between patients, families, medical staff, and the care team to ensure seamless, coordinated care.• Assesses, plans, implements, and evaluates patient care and education needs, documenting interventions and outcomes.• Identifies and addresses barriers to care, assisting with logistical, financial, and psychosocial needs, and referring to supportive services as appropriate.• Coordinates and monitors patient progress, compliance, and transitions of care, providing ongoing feedback and support.• Designs, develops, and participates in the creation and delivery of health education materials and programming for patients and staff.• Maintains advanced specialty training, certifications, licensure, and completes all required competencies and continuing education.• Collaborates with interdisciplinary and community partners to facilitate multidisciplinary care and promote evidence-based practice.• Collects, analyzes, and reports clinical data to support quality improvement, safety, and program goals.• Performs other duties as assigned. ## Education RequirementBachelor’s degree in nursing, or a related fieldMaster’s degree in nursing is preferred ## Experience Requirement0-2 years of experience in nursing, or a relevant area An equivalent combination of education and experience may be considered.All experience must be paid and in the same related field.Part-time and PRN experience will be prorated based on hours worked per week.Volunteer work and internships for academic credit are not counted. ## Certifications & LicensuresRegistered Nurse ## Working ConditionsA.
