Job summary

BETH ISRAEL LAHEY HEALTH is seeking a Hematology/Oncology Nurse Navigator to guide patients from referral to post-treatment follow-up. This role works with a multidisciplinary team to ensure timely access and high-quality care, coordinating same-day visits and triage calls.

The ideal candidate holds a BSN with active Massachusetts RN licensure and BLS certification, and has experience in acute care settings, hematology/oncology preferred. This on-site role requires every-weekday coverage.

Qualifications

• Graduate of a state approved and/or accredited School of Nursing with a Bachelor's degree in nursing.
• Current license to practice professional nursing from the Massachusetts Board of Registration.
• BLS certification.

Responsibilities

Nurse Navigator Hematology/Oncology

Monday through Friday, 4 x 10hr shifts, 7am-5:30pm

The Hematology Oncology Nurse Navigator is the first point of contact for incoming referrals and plays a critical role in ensuring access for new patients. This position works closely with the provider team, to help navigate the patient from initial contact, through diagnosis, treatment, and follow up. The navigators cover our telephone triage line, helping patients with symptom management, and coordinating same day clinic visits when needed. These same day visits allow us to treat patients in the clinic, avoiding visits to the emergency room, and avoiding admission to the hospital. In addition, the Nurse Navigator follows up on Hematology Oncology patients who are discharged from the hospital, to help prevent readmission to the hospital.

Position Summary:

Under the general supervision of the Nurse Navigator Lead in Collaboration with the Nurse Manager, and the Associate Chief of ambulatory nursing practice the Nurse Navigator practices within the Lahey Clinic Professional Practice Model, MA Nurse Practice Act, ANA Standards of Practice and Code of Ethics and demonstrates Lahey Clinic's core values that reflect its Mission. The nurse-patient/family relationship, continuity of care and accountability are central to the delivery of individualized high quality patient care. Works in collaboration with the physician(s), other healthcare provider(s) and other ambulatory support staff within the department and with other disciplines to plan and coordinate the delivery of patient care.

A Nurse Navigator demonstrates the knowledge and skills necessary to provide expected standards of care based on physical, psychosocial, cultural, educational, spiritual, safety and age-appropriate considerations of patients.

The Nurse Navigator, in accordance with the Nurse Practice Act of Massachusetts Rules & Regulations- 244 CMR 3.0, shall bear full and ultimate responsibility for the quality of nursing care he/she provides to individuals and groups. Included in such responsibility are health maintenance, teaching, counseling, collaborative planning and restoration of optimal functioning and comfort or for the dignified death of those they serve.

The Nurse Navigator assumes the responsibility for the plan of care and has the authority to make decisions and is held accountable for decisions specific to assessment, planning, implementation and evaluation of the plan. The registered nurse facilitates the coordination of care across the continuum and demonstrates safe practice within the domains of:

Job Description:

Minimum Qualifications:

• Education: Graduate of a state approved and/or accredited School of Nursing and a Bachelor's degree in nursing.
• Licensure, Certification, Registration: Current license to practice professional nursing from the Massachusetts Board of Registration
• BLS certification.

Pay Range:

$79,268.80 USD - $204,318.40 USD The pay range listed for this position is the annual base salary range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.
