Job summary

Hackensack Meridian Health in Hackensack, NJ, seeks a Nurse Navigator for the Structural & Congenital Heart Program. The role advocates, educates, and facilitates care for CHD patients and families from initial consultation through postoperative evaluation.

The Nurse Navigator, Structural & Congenital Heart Program, is an advocate, educator, and facilitator of care for the Structural and Congenital Heart patient and their family, throughout the continuum of care, form point of initial consultation, through preoperative preparation, admission, hospital stay, and postoperative evaluation.

Education, Knowledge, Skills and Abilities Required:

• Graduate of a NLN/AACN accredited program in nursing.

• RN with at least 5 years of clinical experience in a relevant clinical specialty.

• Proficient computer skills that may include but are not limited to Microsoft Office and/or Google Suite platforms.

Minimum rate of $113,609.60 Annually
