Job summary

The University of Iowa Health Care in Iowa City is seeking a Nurse Clinician/Nurse Navigator to function as a clinical nurse expert and care coordinator in the Emergency Department, guiding transitions across the continuum of care. You will work with an interdisciplinary team to optimize quality care through assessment, education, and follow-up on the plan of care.

Estimated time is 60% with a schedule primarily Monday–Friday and shifts between 7:00 AM and 6:00 PM.

Qualifications

• 3–5 years of clinical nursing experience as an RN.
• Current license to practice nursing in Iowa by date of hire.
• Experience working with an interdisciplinary team.
• Excellent organizational, critical thinking and problem-solving skills.
• Excellent written and verbal communication skills.
• Proficiency in Microsoft Excel, Word, and PowerPoint.

Responsibilities

• Perform transition planning activities to create and update personalized transitional care plans.
• Complete initial assessments to identify barriers to discharge and resource needs.
• Collaborate with providers and health care team to manage length of stay and level of care.
• Assist in discharge planning and referrals for post-acute care services.
• Communicate the plan of care to care team and patients/caregivers to improve satisfaction.
• Intervene to maintain care progression when deviations occur.
• Participate in multidisciplinary huddles and share estimated length of stay and disposition needs.
