Job summary

Confluence Health is seeking a Nurse Navigator to coordinate cardiology care along the treatment continuum. You will provide longitudinal nursing care, manage care plans, and collaborate with the cardiovascular team for optimal patient outcomes.

You will report to the Practice Manager and support virtual care initiatives and care coordination across regions.

Qualifications

• Active nursing license in WA or via compact.
• BLS Health Provider and ACLS cards required.
• Competent in basic computer skills.
• Minimum 3 years of clinical experience.
• Ability to work independently with minimal guidance.

Responsibilities

• Coordinate initial patient plan of care using clinical pathways with the physician and care team.
• Educate patients and families on disease, treatment options, procedures and medications.
• Coordinate tests and referrals with the care team.
• Provide triage and symptom management in person or via telemedicine.
• Maintain active CPR certification and comply with standards.

Summary

The Nurse Navigator is accountable for navigation and coordination of care utilizing evidenced-based clinical pathways for cardiology patients along the continuum of care for their cardiac condition from diagnosis through treatment. They will provide longitudinal comprehensive nursing care to a designated case of cardiac patients. They will have primary nursing responsibilities for the coordination of care, symptom management and education of patients and families. This position will work in collaboration with all members of the healthcare team to achieve the best possible outcomes for the patient.

Position Reports To: Practice Manager

Essential Functions

1. Manage, develop, implement and evaluate initial patient plan of care following clinical pathways in collaboration with the physician and the health care team, to determine the effective utilization of resources and outcome achievement.
2. Facilitate and/or conduct patient, family, and team conferences to develop and monitor a plan of action as necessary. This includes patient education on the disease, treatment options, procedures and medication regimens and side effects.
3. Ongoing assessment of patients for unmet social needs with appropriate referral and communication to social services, community resources, etc.
4. Determine need for further diagnostic work-up; communicate and coordinate necessary tests or procedures to the patient and the healthcare team.
5. Obtain, review and scan outside patient records for new newly referred cardiology patients.
6. Coordinate simple and complex patient care issues in conjunction with other parts of the healthcare system, working interdepartmentally as well as with other members of the cardiovascular team.
7. Determine timeliness/time appropriateness of appointments.
8. Ensures effective and appropriate utilization of resources and efficient patient throughput along continuum of care.
9. Coordinate and continue to develop an established virtual care/telemedicine program for patients in the region, in conjunction with community clinical resources.
10. Assist with patient registry work as it pertains to chronic disease management.
11. Document interim progress notes reflecting coordination of care/services. Intervene when the clinical pathway is not being followed, either due to patient constraints or ineffective follow-through.
12. Provide triage services and symptom management to patients as needed, in-person, telemedicine or over the phone.Communicate symptomology to appropriate team members.
13. Required to maintain active Basic Life Support (CPR) certification.
14. Other duties as assigned.

Qualifications

Required:

• Current licensure in the state of Washington (RCW 18.88) or licensure through Multistate Nurse Licensure Compact (SSB 5499).
• Current BLS Health Provider and ACLS card.
• Competent in basic computer skills.
• 3 years of clinical experience.
• Ability to work independently and perform role with minimal guidance

Desired:

• Previous department experience.

Working Conditions

• Work is performed at a workstation desk and in patient exam/treatment rooms, where space may be limited and confined. Requires a combination of working independently and close communication with providers. Hours may be irregular at times; overtime may be required. General supervision is provided by the Practice Manager with additional instructions, guidance, and direction by the physicians or providers with whom the RN is working.
