Job summary

Trinity Health is seeking a Nurse Navigator to join a multidisciplinary team as an advocate, educator and facilitator of care. The role ensures referred patients receive comprehensive services throughout the continuum and coordinates subspecialty program goals and outcomes.

The Nurse Navigator will assess patients, organize appointments, develop evidence-based care plans, and collaborate with physicians and staff to optimize clinical outcomes and patient satisfaction.

Qualifications

• Bachelor's degree in Nursing required.
• Current Michigan RN license; CPR or BCLS certification required.
• Three years of acute care experience; oncology experience in medical, surgical or radiation oncology preferred.

Responsibilities

• Completes comprehensive health assessment in collaboration with multidisciplinary team.
• Organizes and coordinates timely patient appointments.
• Develops goals and treatment plans with providers based on evidence-based standards and trials.
• Works with physician-led team to review diagnostics and treatment course.
• Collaborates with team on performance improvement and outcome evaluation.
• Uses critical thinking to analyze data and formulate a patient- and family-centered care plan.

Minimum qualifications

What the Nurse Navigator will do:

• Completes a comprehensive health assessment of the patient's physical, psychological, social, environmental,financialandfunctionalstatusincollaborationwithmultidisciplinaryteammembers.Includesassessmentofpastandpresenttreatmentcourse.


• Assistsinorganizingandcoordinatingtimelypatientappointments.


• Understands and develops goals, treatmentplanand provider options in concordance withevidence basedstandards and clinicaltrials.


• Worksdirectlywithphysicianleadtoreviewdiagnosticstudies,clinicalhistoryandcourseoftreatmenttoidentifymostappropriatenextcourse ofcare.


• Collaborateswithmultidisciplinaryteammembersinperformanceimprovementactivitiesincludingassessment,planning,implementationandevaluationofclinicaloutcomesand processesfor patients.


• Usescritical thinking andproblem solvingskills to analyze and synthesize data and formulatean appropriate casemanagementplantoaddressthe patient'sindividual andfamilyneeds.
