Job summary

FMOL Health is seeking a Nurse Navigator to support quality and delivery of patient care across the continuum. You will collaborate with program directors, data analysts, department leaders, staff and patients to drive performance improvement initiatives using LEAN methodologies and evidence-based guidelines.

Qualifications include 3 years in acute clinical settings, a Bachelor’s in Nursing, and an active LA RN license; you must be proficient in English and have strong communication skills.

  • Current and unrestricted Louisiana RN license.

Responsibilities

  • Conducts patient rounds while utilizing clinical expertise to assess and interpret information.
  • Reviews patient encounters, identifying items for performance improvement.
  • Assists program director with maintaining policies and Joint Commission preparation.
  • Coordinates and participates in performance improvement/patient safety forums/committees.
  • Supports new initiatives by identifying and overcoming barriers to clinical care.
  • Serves as active member of workgroups/committees and participates in strategic planning for the Program.

The Navigator also assumes responsibility and accountability for the management of resources to achieve efficient, high-quality outcomes for each patient including support for interdisciplinary and cross facility collaboration. In addition, he/she identifies opportunities to streamline care practice, using LEAN methodology, through development of evidence-based guidelines and protocols.

The Navigator also assumes responsibility and accountability for the management of resources to achieve efficient, high-quality outcomes for each patient including support for interdisciplinary and cross facility collaboration. In addition, he/she identifies opportunities to streamline care practice, using LEAN methodology, through development of evidence-based guidelines and protocols.

Responsibilities
Clinical Practice and Care Management
  • Conducts patient rounds while utilizing clinical expertise to aggregate, assess and interpret information.
  • Reviews Patient encounters, identifying items for performance/process improvement.
  • Assists program director with maintaining policies.
  • Coordinates and participates in performance improvement/patient safety forums/committees.
  • Supports new initiatives by identifying and overcoming barriers to clinical care.
  • Assists with preparation for Joint Commission certification surveys.
  • Serves as an active member of workgroups/committees and is an active participant in strategic planning for the Program. Provides input to meeting agenda, reporting on significant trends and outstanding items.
Communication and Collaboration
  • Serves as a resource/mentor to bedside staff within the service line.
  • Collaborates with multi-disciplinary care team to promote optimal outcomes for patients
  • Partners with program providers to facilitate optimal care supported by guidelines
Quality
  • Responsible for issue identification, issue validation, issue analyses and initiates follow-up to assure loop closure to improve clinical outcomes and patient care.
  • Identify areas of improvement; using this data to develop educational programs, action plans to address quality issues identified and to ensure follow through of developed plans.
  • Facilitate patient education and coordinate follow up care to decrease further complications and decrease readmission rate.
Training and Education
  • Counsels and trains staff members to increase awareness of adverse trends and participation in interdisciplinary process improvement.
  • Identify areas for improvement and education, using PI data to develop education programs to address quality issues identified in the program.
  • Instructs others on pertinent standards and guidelines; ensures these standards and guidelines are adhered to.
