FMOL Health is seeking a Nurse Navigator to support quality and delivery of patient care across the continuum. You will collaborate with program directors, data analysts, department leaders, staff and patients to drive performance improvement initiatives using LEAN methodologies and evidence-based guidelines.
Qualifications include 3 years in acute clinical settings, a Bachelor’s in Nursing, and an active LA RN license; you must be proficient in English and have strong communication skills.
The Nurse Navigator provides assistance in the improvement of the quality/delivery of patient care throughout the continuum of care. He/she works in collaboration with the program directors, data analysts, department leaders, staff and patients to develop and support the performance improvement initiatives. The Navigator also assumes responsibility and accountability for the management of resources to achieve efficient, high-quality outcomes for each patient including support for interdisciplinary and cross facility collaboration. In addition, he/she identifies opportunities to streamline care practice, using LEAN methodology, through development of evidence-based guidelines and protocols.
