Job summary

Qualifications

• Ability to coach, train and provide staff development.
• Proficiency in Microsoft Word, Excel, and PowerPoint.
• At least five years of previous home health care experience.

Responsibilities

• Supervise all HomeVisits RN’s.
• Complete the MDS-HC and other clinical assessments.
• Monitor health status of all members for proper AFC delivery.
• Conduct admission on-site visits for new members.
• Train and supervise AFC/GAFC caregivers.
