Job summary

The Veterans Health Care System of the Ozarks seeks a Registered Nurse (Nurse Manager) for Home Based Primary Care Services. The nurse leader oversees nursing care, coordinates patient education, and promotes self-management and patient-centered outcomes across the continuum of care.

Responsibilities include leading staff orientation and development, collaborating with an interdisciplinary team, and advancing evidence-based practices aligned with VA goals.

Qualifications

• Active nursing leadership experience in a healthcare setting.
• Experience coordinating Home Based Primary Care or similar programs is a plus.
• Strong collaboration with interdisciplinary teams to improve patient outcomes.

Responsibilities

• Oversee orientation, training, and staff development for HBPC nurses.
• Direct and supervise nursing care and ensure adherence to nursing process.
• Collaborate with interdisciplinary team to remove barriers and improve care quality.
• Align nursing practices with VA strategic goals and evidence-based care.
• Provide patient education and support for self-management and care continuity.
