Job summary

This on-site, full-time role requires a BSN, California RN license, and at least five years in healthcare with three years in case management, plus leadership experience.

Qualifications

• At least 1 year of healthcare leadership experience is required
• CCM or ACM certification preferred

Nurse Manager Case Management Job Requirements

• Bachelor’s Degree in Nursing (BSN) required; Master’s Degree in Healthcare or related field preferred.
• Active California Registered Nurse License upon hire is mandatory.
• Minimum of 5 years of experience in a healthcare role, with 3 years in case management within an acute care setting.
• At least 1 year of healthcare leadership experience is required.
• Certified Case Manager (CCM) or Accredited Case Manager (ACM) certification preferred.
• Strong knowledge of care coordination processes, social determinants of health, and resource management.
• Excellent communication, leadership, and organizational skills to manage a multidisciplinary team effectively.
• Ability to analyze data and implement strategies to improve patient outcomes and operational efficiency.
