Job summary

Evolve Treatment Centers in California is hiring a Full-time LVN/LPT to oversee our two facilities in Agoura Hills and Calabasas. You will provide direct patient care to adolescents, perform assessments, and participate in treatment planning with a multidisciplinary team.

The ideal candidate holds a valid California LVN/LPT license, CPR/BLS, and at least one year of nursing experience, preferably in psychiatry or chemical dependence.

Qualifications

Responsibilities

• Direct patient care and assessment; report changes in mental/physical condition
• Complete nursing documentation and MD orders

• Participate in treatment planning and attending staff meetings
• Call in medications to pharmacy and coordinate pickups
• Provide in-service training on medications and monitoring to staff
• Educate patients and families about medications and health issues

Evolve Treatment Centers is hiring for a Full-time LVN/LPT to oversee our 2 facilities in Agoura Hills and Calabasas CA! Our team provides cutting edge residential and outpatient treatment to adolescents struggling with addiction and mental illness. Our rapidly growing company is looking for those who are highly qualified and passionate mental health professionals to join our team!

Our ideal candidate is passionate about working with teens and has strong communication skills, time management skills, and organizational skills. Candidates must be eager to participate in an environment that promotes continuous learning and works as a member of a team. We are looking for candidates that can stay calm during stressful situations, those that take initiative and maintains appropriate boundaries with clients.

Essential Functions:

• Direct patient care and assessment; including the reporting of observed changes in mental status and/or physical condition, response to emergencies, and safety risks
• Completes documentation in an efficient and timely manner, including nursing assessments, transcription of M.D.s' phone orders
• Knowledge of medication administration records, vital sign monitoring
• Attendance and participation in terms of treatment planning and professional staff meetings
• Call in medications to pharmacy and pick up
• Provides in service training to technical staff on medications, documentation, and monitoring of vital signs
• Provides ongoing patient education on all aspects of medications and other health issues including hygiene
• Monitors supplies and reports needs to Facility Manager
• Establishes a compassionate environment by providing emotional, psychological, and educational support to patients and families
• Resolves patient problems and needs by utilizing multidisciplinary team strategies
• Maintains safe and clean working environment by complying with procedures, rules, and regulations; calling for assistance from health care support personnel
• Protects patients and employees by adhering to infection-control policies and protocols, medication administration and storage procedures
• Maintains continuity among nurses by documenting and communicating actions, irregularities, and continuing needs to weekend nurse

Knowledge of:

• Psychiatric medications and their side effects
• Basic first aid and life‑saving skills

Ability to:

• Speak with clients and parents in a compassionate and patient manner
• Maintain professional boundaries with clients and staff
• Assess emergency situations and delegate responsibilities to staff

Requirements:

• Valid California Nursing License LVN/LPT
• Current CPR/ BLS card

This is a Full‑time, onsite position, Mon - Fri 8:30 am - 5pm.

Payrate: $70,000 - $77,000 annual
