Job summary

OhioGuidestone is seeking a part-time Licensed Practical Nurse or Registered Nurse to support client care across Ohio locations, including Berea, Summit, and Medina offices. The role involves updating charts, performing basic diagnostics, and collaborating with therapists and program managers.

The ideal candidate holds an Ohio LPN license (RN preferred), with experience in mental health/substance use settings and proficiency in EHRs.

Qualifications

• Must hold State of Ohio LPN license or RN license; RN preferred.
• Associate's degree in Nursing preferred; HS diploma required.
• Experience with mental health/substance use settings is a plus.
• Proficient with electronic health records and Microsoft Office.

Responsibilities

• Update and maintain client charts.
• Provide diagnostic tests as directed.
• Communicate with therapists, program managers, and off-campus programs.
• Provide ongoing education on infectious disease, vitals, and medications.
• Carry out orders from doctors and nurse practitioners.
• Monitor vital signs and report to clinicians.
• Assist with referrals to health professionals.
• Rotate shifts as needed based on staffing.

Part time 16 hours/ week

(might need to travel to Berea, Summit and Medina offices)

Essential Functions:

Demonstrate the values and guiding principles of OhioGuidestone, performing with the highest standards of excellence, integrity and ethics.

Update and maintain client charts

Provide diagnostic tests as directed and/or needed

Establish and maintain an open line of communication with all therapists, program managers, and off campus programs

Provides ongoing education for infectious disease, Hepatitis, vitals, and medications for employees

Attend nursing meetings as scheduled

Carry out orders as received from doctors and/or nurse practitioners

Monitor height, weight, blood pressure, and pulse of clients and reports to doctors and/or nurse practitioners

May be involved in the planning, organization, and implementation of special events, specific projects, or program changes

Demonstrate positive leadership, promote a team-based work environment and a culture of belonging.

Exhibit positivity, flexibility and a willingness to take on new responsibilities as requested or required.

Present the Agency in the most positive light with all internal and external contacts.

Complete prior authorizations for medication and return phone calls to clients/families prior to end of shift

Coordinate the medical portion of client care while receiving services at OhioGuidestone

Complete nursing assessments for clients

Assist with referrals for clients to see health care professionals

Rotate shift or site as need based on staffing needs

Provides ongoing education on mental health diagnosis and medications

Education & Experience:

High School Diploma/GED required; Associate's degree in Nursing preferred

Valid State of Ohio Licensed Practical Nursing required, Registered Nurse license preferred

Experience and/or knowledge with mental health and substance abuse within children, adolescents and adult population preferred

Outpatient office experience recommended

Must be able to access work locations, offices, and homes as necessary, which may include ascending/descending stairs.

Possess a valid Ohio Driver’s License with a safe driving record and valid insurance.

Successfully complete pre-employment screenings, including drug tests, background checks, and any other required assessments, such as a physical examination and TB test.
