Trinity Social Services, LLC seeks a local traveling LPN to support our Behavioral Health program. The nurse will function as an independent care provider, focusing on managing patients’ mental health and behavioral needs.

Responsibilities include assessments, documentation, medication administration, and collaboration with physicians and a multidisciplinary team. Travel is required to visit clients, educate families, and perform routine follow-up visits.

Qualifikationen

• Current GA State licensure as an LPN is required.
• American Heart Association BLS certification.
• ACLS certification.

Trinity Social Services, LLC is seeking a local traveling LPN nurse for our Behavioral Health program. This nurse will be an independent care provider, highly skilled in managing patients' mental health and behavioral needs.

Primary Responsibility:

Promotes and restores clients’ health by completing the nursing process, collaborating with physicians and multidisciplinary team members, providing physical and psychological support to clients, friends, and families, and supervising assigned team members.

Job Duties and Responsibilities:

• Providing nursing assessment, AIMS, and interventions to observe, monitor, and care for physical, nutritional, behavioral health, and related psychological issues, problems, or crises manifested in the course of the client’s treatment.
• Maintaining accurate, complete health care records and reports
• Administering medications to clients and monitoring them for side effects and reactions
• Recording patient vital signs and medical information
• Ordering medical diagnostic and clinical tests
• Monitoring, reporting, and recording symptoms or changes in patient conditions
• Administering non-intravenous medications
• Assessing, implementing, planning, or evaluating patient nursing care plans by working with healthcare team members
• Modifying patient health treatment plans as indicated by patient conditions and responses
• Clinic schedule 1x-2x monthly
• Call in medication ordered by NP/PA
• Meet with clients/guardians to educate on NP/PA instructions, meds, etc., after meds are called in, and to determine questions
• Conduct a routine follow-up nursing session

Requirements:

• Current GA State licensure as an LPN nurse is required
• American Heart Association BLS
• ACLS certificationAbility to effectively communicate with clients, families, physicians, and hospital staff

• Experience in mental health or home health care (plus)
• Experience with Carelogic (plus)
