Job summary

Central Valley Regional Center (CVRC) in Fresno, CA is seeking a Nurse II to perform nursing assessments, health evaluations, and consult with service coordinators to identify health service needs. The role includes case reviews, interagency liaison, and participation in health-related clinics within the CVRC network.

Qualifications

  • Current valid licensure in California as a Registered Nurse (RN) and two years of experience in developmental disabilities or related field.
  • Understanding of disabilities and related conditions.
  • Skilled in clinical and public health needs and interventions.
  • Ability to establish and maintain relationships with others.
  • Ability to prioritize varied work assignments and complete documentation timely.
  • Ability to communicate effectively verbally and in writing.
  • Must have a valid CA driver’s license, reliable vehicle, and vehicle insurance.

Responsibilities

  • Assesses served individuals nursing and health needs. Assists with examinations, tests, or procedures to clarify diagnoses.
  • Provides recommendations to staff, individuals served, parents, community, and program personnel.
  • Consults with CVRC staff, including BCBA’s, psychologists, physicians and pharmacists, to triage cases.
  • Co-ordinates and participates as nurse consultant at clinics; signs off on eligibility; follows up with insurance/DME.
  • Reviews medical records and provides necessary follow-up.
  • Codes diagnoses in SANDIS database and on diagnostic sheet.
  • Serves as consultant to staff regarding facilities and health services quality.
  • Visits health and safety to facilities, homes, and hospitals as needed.
  • Prioritizes urgent needs and documents/relays information to service coordination.

Central Valley Regional Center (CVRC) is a non-profit corporation partnering with the State of California Department of Developmental Services. CVRC provides case management and service coordination by identifying resources, programs and activities to refer individuals to in collaboration with other agencies to develop the best services for individuals served and families. CVRC serves individuals with intellectual and developmental disabilities, and children at risk to support and promote community inclusion. CVRC provides case management for individuals residing in the Central Valley, covering six counties that include Fresno, Tulare, Merced, Kings, Mariposa, and Madera.

POSITION OVERVIEW

This position provides nursing assessments and evaluations, consultation on health issues to service coordinators, and reviews cases and identifies appropriate health service needs. Participates in community resource access and liaison tasks.

ESSENTIAL FUNCTIONS

  • Assesses served individuals nursing and health needs. Assist when necessary for specific examinations, tests, or other procedures to clarify diagnoses.
  • Provides recommendations to staff, individuals served, parents, community, and program personnel. Participates in planning process to meet identified needs.
  • Consult with various CVRC staff such as the BCBA’s and psychologists and contracted physicians and pharmacist, to triage cases.
  • Coordinates and participates in the role of a nurse consultant at various clinics at CVRC (Physiatry/Rehabilitation Clinic, Early Intervention Clinics, etc.) Will sign off on eligibility when needed. Follow-up with insurance and DME companies on specific equipment orders.
  • Reviews medical records received and provides necessary follow up when needed.
  • Assists in acquiring information to ensure records are maintained and updated. Code diagnoses in the SANDIS data base and on the individual’s diagnostic sheet.
  • Serves as a consultant to service coordinators regarding facilities and provide a health assessment to evaluate the quality of health services provided to individuals served.
  • Visitations for health and safety to facilities, homes and hospitals when necessary.
  • Prioritizes and responds to urgent needs of individual’s served and their families as required and appropriately documents and/or relays information to service coordination.
  • Review medical SIRs, participate in SIR Reviews, participate in Corrective Action Plans (CAPs), and mandated reports to other agencies as appropriate (e.g., Adult Protective Services (APS), Community Care Licensing (CCL) and Department of Child and Family Services (DCFS).
  • Reviews, approves and signs off on Restricted Health Care Plans
  • Represents CVRC at case conferences and interagency meetings. Chairs or participates on committees related to health needs.
  • Identifies, coordinates, and provides information on individuals served benefits and services.
  • Supports effective and efficient business practice.
  • Maintains positive business relationships, confidentiality, good attendance, punctuality, and acts in accordance with all company policies and procedures.

MINIMUM QUALIFICATIONS

  • Current valid licensure in California as a Registered Nurse (RN) and two (2) years of experience in developmental disabilities or related field.

  • Ability to establish and maintain comfortable and effective relationships with others.
  • Ability to prioritize varied work assignments and complete documentation within required timelines.
  • Ability to communicate effectively verbally and in writing.
  • Ability to use computer and software for documentation, the management information systems, e-mail, and other required computerized functions.
  • Must demonstrate initiative, self-motivation, sound independent judgment, flexibility, strong interpersonal, and excellent written and oral communication skills.
  • Must have a valid CA driver’s license, reliable vehicle, and vehicle insurance.

PREFERRED QUALIFICATIONS

  • Knowledge of generic public resources and developmental disabilities desirable.

TRAVEL REQUIREMENTS

  • Must have the ability to travel within CVRC’s six-county-wide region as assigned or required.

WORKING CONDITIONS

Professional office environment. Noise level in the office is low to moderate depending on area. Individual workspace is assigned and varies from open cubicle to private office.
