Job summary

Acenda Integrated Health in New Jersey is seeking a Nurse Home Visitor to support the Nurse Family Partnership in Atlantic County. The role involves traveling to clients' homes to provide education and support from pregnancy through the child's second birthday, building trust with families.

You will help improve pregnancy outcomes, child health, and family self-sufficiency while connecting clients to community resources and ensuring adherence to the NFP model.

Qualifications

• BSN required.
• Active NJ Registered Nurse license (in good standing).
• Must have and maintain a valid driver's license, use of an insured vehicle and an acceptable driving record.
• Professionalism and a commitment to excellence in care.

Responsibilities

• Travel to clients' homes and provide home visits to first‑time mothers providing education and support through pregnancy and up until the child's 2nd birthday.
• Work to improve pregnancy outcomes, child health and development outcomes, and the economic self‑sufficiency of families.
• Build long‑term therapeutic relationships with clients and their families.
• Provide community resources as needed per nursing assessment.
• Adheres to the nursing process and the Nurse‑Family Partnership model of home visitation.
• Attends and participates in case conferences and team meetings.
• Collaborates with client and family to develop short‑and‑long‑term goals.
• Formulates nursing diagnosis based on nursing assessment, client goals and Nurse‑Family Partnership outcomes.
