LAUKOA is seeking a Registered Nurse Case Manager to oversee continuity of care for Service Members via the Virtual Medical Center (VMC). The role involves coordinating assessments, developing care plans, and collaborating with physicians, payers, and families to optimize outcomes.

The nurse will educate patients and teams, ensure timely documentation, and participate in process improvements to enhance care quality and cost effectiveness.

Qualifications

• Bachelor's degree in Nursing from ACEN/CCNE-accredited program.
• Must hold one of multiple certifications: CCM, CDMS, CRRN, COHN/COHN-S, ACCC, CRC, RN-NCM, or CMC.
• One year of nursing experience post-graduation.
• Active, unrestricted RN license.
• No board certification required.

Responsibilities

• Provide clinical nursing, administrative and organizational skills to manage continuity of care for beneficiaries served by the VMC.
• Coordinate follow-up consults and care services for Service Members.
• Collaborate with multidisciplinary teams to develop treatment timelines and care plans.
• Develop and implement local strategies and tools to support case management and documentation.
• Maintain electronic health records and ensure timely updates to patient charts.
