Job summary

Incredible Health in Louisville, CO is hiring a Nurse Case Manager (RN) to coordinate patient care and ensure smooth transitions across the care team. You will conduct comprehensive assessments and collaborate with clinicians to develop individualized plans.

Required BSN or higher, active RN license, 1+ year in case management, and strong EHR skills. Day and night shifts are available; both full-time and part-time opportunities with comprehensive benefits.

Responsibilities

• Conduct comprehensive patient assessments including history, medications, treatment plans, and psychosocial needs
• Collaborate with healthcare providers, patients, and families to develop and implement individualized care plans
• Monitor and evaluate patient progress and adjust care plans as needed
• Serve as main point of contact between patient and healthcare providers for seamless communication
• Maintain up-to-date records and case notes with confidentiality and compliance

Qualifications

• Bachelor of Science in Nursing (BSN) or higher educational attainment from an accredited program
• Active and unencumbered Registered Nurse license
• 1+ years experience in case management, preferably within a healthcare or hospital setting
• Exceptional interpersonal and communication skills, both written and verbal, to effectively collaborate with medical professionals, patients, and family members
• Proficiency in electronic health records (EHR) software

Responsibilities
