Job summary

Generations Healthcare in La Mesa, CA is seeking a Nurse Case Manager (LVN) with skilled nursing facility experience. This full-time role from 9a-5p offers wage between $42-$46 per hour, based on experience.

The incumbent will develop and manage patient care plans and coordinate with families and providers to ensure quality care. Responsibilities include evaluating plans, assessing new patients, reviewing test results, implementing orders, and maintaining documentation.

Qualifications

• Must possess, as a minimum, a Nursing Degree from an accredited college or university.
• Must have, as a minimum, one (1) year of experience as a supervisor in a hospital, nursing care facility, or other related health care facility.
• Must have training in rehabilitative and restorative nursing practices.
• Strong communication, excellent customer service, teamwork and organizational skills
• Must possess a current, unencumbered, active license to practice as an RN in this state.
• Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.

Responsibilities

• Evaluate and update patient health care plans
• Assessment for new patients
• Review test results from medical exams
• Implement Physician orders and follow up with treatment plans
• Communicate with families, health providers and patients
• Documentation of medical records
• Provide emotional support to families and patients
• Provide education about health care plans to patients and families
