You will collaborate with the care team to anticipate discharge needs, understand medical necessity and planned level of care, and connect patients with community resources.

Qualifications

• Bachelor's Degree in Nursing required; Master's degree preferred.
• 3 years of relevant clinical nursing experience required.

Responsibilities
