Prairie Wind Hospice is seeking a Nurse Case Manager to coordinate and provide exceptional hospice care to patients and families. The role works with the Director of Nursing and the Interdisciplinary Team to deliver hands-on nursing, assess pain and symptom management, and support dignified end-of-life care.
You will educate families, participate in interdisciplinary meetings, update the Plan of Care, and coordinate services to meet patient goals while upholding the hospice philosophy.
Skilled Nurse from an Accredited program currently licensed and in good standing with the Kansas State Board of Nursing. One to 2 years’ experience in nursing in an acute care setting or 1-year experience in home health, hospice or comparable experience is preferred. Desire to learn and demonstration of aptitude for the hospice philosophy and comfort care required.
It is the responsibility of the Nurse Case Manager to coordinate and provide the highest level of hospice care to patients and families.
The Nurse Case Manager works in conjunction with the Director of Nursing (DON) and the Interdisciplinary Team to:
Prairie Wind Hospice will provide orientation and training, as necessary, to assist the Case Manager Nurse in performing the responsibilities listed above. Training may be in-house or outside seminars. Evidence of orientation and training will become part of the personnel file.
The Case Manager Nurse will not be on call at this time as we have a full on call team.
Reports directly to the Director of Nursing. The Nurse Case Manager will work under the direction of the Director of Nursing.
