Health Wealth Safe, Inc. seeks a patient‑centered care coordinator to guide patients and families across the care continuum.

You will assess needs, coordinate services, and support transitions between home health, clinics, and community resources. This role includes educating families for understanding and adherence, collaborating with an interdisciplinary team, and supervising LPNs and home health aides to ensure quality and safety.
