UpstreamManagement is hiring a Nurse Case Manager in Oklahoma City to oversee case management of assigned patients and provide compassionate care. The ideal candidate will have 1–2 years of home health nursing experience and a current Oklahoma RN license.
This role involves developing patient care plans, providing skilled nursing interventions, and ensuring compliance with healthcare regulations. We seek someone with exceptional communication, compassion, and critical thinking skills.

Under the supervision of the Director of Clinical Services, the Nurse Case Manager is responsible for the case management of assigned patients in accordance with the agency policies and all applicable laws and relevant professional standards. The (Registered) Nurse Case Manager plans, organizes, and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. This position may also require field nurse coverage at the direction of the Nurse Administrator.
This is an Exempt position that reports to the Director of Clinical Services.

Specific vision abilities required by this job include close vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
The incumbent must be able to effectively operate the current computer system, telephone system, and other office machines such as a copier.
