Job summary

LanceSoft is seeking a Nurse Case Manager in Las Vegas, NV to coordinate patient care across the continuum. This full-time role partners with physicians, nurses and care teams to assess needs, plan care, and ensure smooth transitions from admission to discharge.

Requires a Nevada RN license, active BLS, and at least 3 years in acute care, plus 1 year in case management or discharge planning. Strong communication and care coordination skills are essential to support quality outcomes.

Qualifications

• Graduate from an accredited nursing program
• Active Nevada RN license
• BLS certification (AHA)
• Minimum 3 years of acute care nursing experience
• At least 1 year of case management, discharge planning, or utilization review experience
• 3+ years of case management experience in acute care (preferred)
• Certifications such as CCM or ACM (preferred)

Position Summary:
We are seeking an experienced Nurse Case Manager to coordinate patient care across the healthcare continuum. This role involves working collaboratively with multidisciplinary teams to assess, plan, and manage patient care, ensuring optimal outcomes.
