Job summary

Zepf Center, a leading behavioral health provider in Northwest Ohio, seeks a Care Navigation Coordinator to improve outcomes for prenatal and postnatal patients with opioid use disorder. You will educate patients, coordinate care, and link services internally and externally to support sustained recovery and healthy mother-child outcomes.

In this role you will work with a multidisciplinary team, document progress in the EHR, and ensure timely access to OB/GYN services, MAT programs, and community

Qualifications

  • LPN or RN required; bachelor’s degree in nursing, social work, community health, or a related field preferred
  • 3–5 years of related experience required
  • Chemical Dependency licensure preferred
  • Must have a valid driver’s license and insurance
  • Preferred knowledge of OMHAS, DEA and CARF standards
  • CPR and First Aid trained, and CPI trained

Responsibilities

  • Provide patient education and treatment access for patients using SBIRT (Screening, Brief Intervention and Referral to Treatment model)
  • Maintain internal and external contacts including community-based organizations to support diverse patient needs
  • Serve as the liaison between the hospital and the Zepf Center for both the patient and the baby
  • Coordinate multidisciplinary team meetings that support patient needs, and develop a plan of care that best serves the needs of the individual patient
  • Assess, link, monitor, advocate and case plan for the patient
  • Knowledge of current federal, state and county reporting guidelines for substance-exposed infants
  • Match treatment response to each woman’s stage of recovery and/or readiness to change
  • Collect and maintain data necessary for reporting and compliance activities
  • Coordinating with the authorizing and health care or social service entities as necessary to maintain and support the patient's goals
  • Facilitate successful transition of care from community partners to inpatient services and MAT outpatient settings
  • Coordinate scheduling of Zepf patients to OB/GYN services with ProMedica
  • Document patient progress and recommendations in the EHR; all in person and telephonic encounters will be documented
  • These services shall be provided in either a community or home setting
  • Document and bill for all program as allowed according to the agency’s established deadlines
  • Assist with co-facilitation of weekly groups
  • Perform other duties as assigned

General Summary

The purpose of this position is to improve patient outcomes for prenatal and postnatal patients with opioid use disorder through education programs, treatment, case management, linkage to programming both internally and externally, coordination of care and assessments. The Care Navigation Coordinator will assist these patients to improve their overall functioning with identification of strengths, weaknesses, abilities and preferences in order to attain future goals


Essential Duties And Responsibilities


  • Provide patient education and treatment access for patients using SBIRT (Screening, Brief Intervention and Referral to Treatment model)
