Job summary
Saint Francis Healthcare System is seeking a Nurse Care Manager (RN) to provide expanded care for Health Home participants, promoting best interests of the program, care team, and patients.
The role is full time (80 hours per 2 week pay period) with benefits; typical hours are Monday–Friday 8:00am–4:30pm; Missouri-licensed RN required; BSN preferred.

Qualifications
- Must be licensed as a Registered Nurse in Missouri.
