Position Summary
The Nurse Care Manager (RN or LPN) is responsible for coordinating and managing patient care services to improve health outcomes, promote preventive care, and support patients with chronic and complex medical conditions. The Nurse Care Manager serves as a liaison between patients, providers, specialists, community resources, and insurance partners to ensure continuity of care.
Essential Duties and Responsibilities
- Assess patient needs and identify barriers to care.
- Develop, implement, and monitor individualized care plans.
- Coordinate care among primary care providers, specialists, hospitals, and community resources.
- Manage high-risk and chronic disease patient populations.
- Conduct patient outreach and follow-up after hospitalizations, emergency room visits, and specialty appointments.
- Provide patient education regarding disease management, medications, preventive care, and healthy lifestyle choices.
- Track quality measures and assist with value-based care initiatives.
- Maintain accurate and timely documentation in the electronic health record (EHR).
- Collaborate with providers and interdisciplinary teams to improve patient outcomes.
- Ensure compliance with HIPAA, clinic policies, and regulatory requirements.
Qualifications
- Current Registered Nurse (RN) Licensed Practical Nurse (LPN) license in Missouri.
- Previous experience in care management, case management, population health, family practice, or community health preferred.
- Strong assessment, organizational, and critical-thinking skills.
- Ability to build rapport with patients and families.
- Experience with EHR systems and documentation.
- Knowledge of chronic disease management and care coordination principles.
- Excellent communication and customer service skills.
Preferred Experience
- Federally Qualified Health Center (FQHC) experience.
- Experience working with Medicaid, Medicare, and underserved populations.
- Care Management or Case Management certification (preferred but not required).
- Full-time, Monday through Friday.
- Clinic-based position serving Anderson MO location.
- No routine weekends, holidays, or on-call responsibilities.
