Responsibilities
  • Provide direct hands‑on resident care with all Activities of Daily Living (ADLs).
  • Perform clerical, receptionist, and communication duties at Pole Creek Estates under the direction of nursing and medical staff.
  • Utilize individual care plans of residents for the performance of resident care.
  • Attend to individual needs of residents, which may include assistance with grooming, bathing, oral hygiene, feeding, incontinent care, toileting, colostomy care, and prosthetic appliance.
  • Participate in creating and following through with the specific care plan of each resident, i.e. all ADLs, and post‑mortem care. Observe and report any change in resident’s vital signs or status to nurse caring for resident or charge nurse.
  • Serve and collect food trays, pass water, and assist residents with feeding who require assistance. Accurately record a resident’s intake and output.
  • Assist in setting up auxiliary equipment (i.e. oxygen, suction, Gomco, nebulizers, K‑Pads, etc.) under the direction of the supervising nurse. Keep resident rooms neat, clean, and free of obstructions. Apply compresses, ice bags, aqua‑pads, and Ted hoses.
  • Lift, move, and transport residents using proper body mechanics or lifting devices.
  • Provide care that maintains each resident’s skin integrity to prevent pressure ulcers, skin tears, and other damage by changing incontinent residents, turning or repositioning immobile residents, and applying moisturizers to fragile skin and other areas.
  • Perform and maintain documentation.
  • Observe residents for changes in condition and behavior and promptly report these changes to appropriate licensed nursing personnel.
  • Communicate and collaborate effectively with other health team members regarding resident condition, nursing plan of care, resident satisfaction needs, and recommendations for meeting identified outcomes.
  • Communicate and interact effectively and tactfully with residents, visitors, families, peers, and supervisors.
  • Promote and ensure resident safety in performance of all responsibilities.
  • Other duties as assigned by management.
