Job summary

3HC Home Health & Hospice Care Inc. in Cumberland, Duplin and Sampson County, North Carolina, is seeking a Certified Nursing Assistant I to provide personal care and related services under a qualified professional's plan of care.

The CNA will follow the patient's individualized plan, administer personal care, record vitals, and participate in staff orientation while ensuring compassionate, one-on-one care in a home health setting. Valid NC driver's license required.

Qualifications

Responsibilities

• Creates positive experiences for customers meeting their expectations with courtesy, tact and knowledge of agency services.
• Administers personal care and hygiene per the patient's care plan.
• Takes and records vital signs and weight; reports changes to RN or Director to prevent adverse events.
• Participates in orientation of new staff to patients, family, and home setting as requested.
• Submits documentation via MobileCare system and time/travel sheets accurately and timely.

Summary:

The Certified Nurse Aide I is responsible for the personal care and related services to home health, hospice, and home care patients as outlined on the patient's individualized plan of care delegated by the qualified professional.

CNA Essential Job Functions:

1. 1. Creates positive experiences for internal and external customers that will meet their expectations. Displays a high degree of courtesy, tact, and knowledge of services provided by the agency in all contact with staff, patients, and visitors.
2. 2. Administers personal care and hygiene by following the patient's individualized plan of care.
3. 3. Accurately takes and records vital signs and weight, as ordered on the patient's individualized plan of care. Immediately reports and records changes in the patient's mental or physical condition to RN or Clinical/Assistant Director to prevent adverse events.
4. 4. Actively participates in the orientation of new staff to patients, family members, and the home setting as requested.
5. 5. Submits documentation as evidenced by reports: (a) utilizes the MobileCare system daily to verify schedules and care plans; (b) enters progress notes as outlined in policy 90% of the time; (c) enters employee time via MobileCare system accurately and timely; (d) submits accurate time and travel sheets at least weekly; and (e) submits other data to supervisor as requested on a timely basis.

Requirements for Hospice Nursing Assistant:

1. 1. Listed as a CNA with DHSR, as evidence by successfully completed training & has not had a lapse of 24 consecutive months where the aide has not provided services for compensation.
2. 2. Current CPR certification.
3. 3. Ability to read and write.
4. 4. Must be dependable and capable of dealing pleasantly and tactfully with patients, family, staff, and the public.
5. 5. Possess valid North Carolina Driver's License and available means of transportation.
