Job summary

Carlton Senior Living is seeking an Onsite Nurse to provide direct care and medication administration in our Assisted Living and Memory Care communities.

You will monitor residents' health, document changes, and coordinate with physicians, families, and a care team to ensure quality, compliant care. The role includes supporting Personal Service Plans and emergency responsiveness.

Qualifications

• Must be at least eighteen (18) years or older per Title 22 requirements.
• Current nursing license required; must be in good standing and free of disciplinary action
• At least one year of experience preferred but not required

Responsibilities

• Provide direct care and administer medications and treatments to residents in Assisted Living and/or Memory Care in accordance with physician orders and state regulations.
• Monitor residents' health conditions, vital signs and behavior; document and communicate changes to physicians, families and team members promptly and accurately.
• Support the development and ongoing updates of residents' Personal Service Plans, coordinating care with interdisciplinary teams and external providers.
• Perform medical tasks such as wound care, catheter care, oxygen administration and injections as permitted by licensure and community standards.
• Supervise and monitor medication administration including transcriptions, narcotic counts and proper storage/disposal.
• Respond to emergencies, manage family and resident concerns professionally and maintain shift documentation for quality and compliance.
• May assist residents with activities of daily living including continence care, showering, grooming, transfer assistance and meal assistance.

What Youll Do
