Job summary

Greenfield Rehabilitation and Nursing Center is seeking an RN/LPN to join its team in Greenfield, MA. The role involves delivering nursing care, administering medications, and assisting physicians with procedures in a supportive, patient‑centered environment.

Qualifications

• Active RN or LPN license in good standing.

Responsibilities

• Provides nursing care to patients as assigned.
• Administers and records medications and treatments as assigned.
• Prepares for and assists Physicians with diagnostic procedures and treatments.
• Reinforces health care instructions to patients and families.
• Collaborates with the care team in planning and evaluating nursing care.
• Assists with admission, transfer, and discharge of patients.
• Monitors vital signs and reports changes to the care team.
• Documents patient responses to care in charts.
• Maintains safe and comfortable patient environments.
• Participates in unit management and may assume charge when needed.

Duties and Responsibilities:

• Provides nursing care to patients as assigned.

• Administers and records medications and treatments as assigned.

• Prepares for and assists Physicians and others with diagnostic procedures and treatments.

• Reinforce supports and provides health care instructions to patients and their families.

• Participates with other team members in planning, implementing and evaluating nursing care and other patient activities.

• Assists in the admission, transfer, and discharge of patients.

• Observes and reports patients’ vital signs, symptoms and general condition to the appropriate individual(s).

• Records on patient charts observation of patient’s responses to nursing care rendered.

• Maintains environmental conditions conducive to the comfort and safety of patients.

• Assists with unit management and/or assume charge of unit as assigned.

Experience:

• RN/LPN Registered Nurse Licensed Practical Nurse:1 year (Preferred)

License:

• RN/LPN Registered Nurse Licensed Practical Nurse(Required)
