Job summary

Altea Healthcare is seeking a Certified NP or PA-C to join our post-acute care team in Burlington, WI. You will diagnose, treat, and guide patients in a skilled nursing facility, with emphasis on geriatric, internal medicine, and primary care.

We offer a highly competitive base salary ($135K–$160K) with uncapped bonuses, full benefits, 401(k) match, ongoing education, and mentorship for new grads and experienced providers. Relocation required.

Qualifications

• Must have NP or PA-C certification and current licensure.
• Experience with geriatric or post-acute care is preferred.

Responsibilities

What You’ll Do:

As a Certified NP or PA-C, you will be a key clinical provider in a skilled nursing facility, diagnosing, treating, and guiding patients to better health. No two days are the same!

Your daily responsibilities include:

• Performing physical exams and reviewing medical histories.
• Ordering and interpreting diagnostic tests (labs, imaging, etc.).
• Diagnosing and managing acute and chronic conditions.
• Prescribing medications and creating treatment plans.
• Collaborating with physicians, nurses, and facility staff.
• Educating patients and families on health conditions and preventive care.
• Documenting patient care accurately and efficiently.
